Forrest Henry McCullough was born to Benjamin Irvin McCullough (1896 - 1963) and Jessie Gertrude Snyder (1903 - 1991). His father was born in Alabama, as was his mother. He had siblings Benjamin Whitfield and Eleanor Frances McCullough and half-siblings Thelma Mae, Willie Edna, Laverne Joseph, and Robert Lee McCullough. Forrest McCullough married Alyce Jane Ledford on June 25, 1954 in Coffee, Tennessee. Both bride and groom were 19 years old. They had a daughter, Mrs. Donald Hawkins, and a son, Forrest H. McCullough II. A former evangelist for the Church of the Nazarene and the owner of the Evangelistic Bookstore, Forrest died at the age of 39 after having suffered a self-inflicted gunshot wound. Rev. Forrest McCullough TULLAHOMA - Services for the Rev. Forrest Henry McCullough, 39, of Tullahoma, owner of Evangelistic Bookstore and a former Church of the Nazarene minister, will be at 11 a.m. tomorrow at First Baptist Church. Burial will be in Maplewood Cemetery. The Rev. Mr. McCullough died Tuesday at his home of a self-inflicted gunshot wound, according to the Franklin County coroner. The body is at Daves-Culbertson Funeral Home. Survivors include his widow, Mrs. Jayne Ledford McCullough; a daughter, Mrs. Donald Hawkins, Nashville; a son, Forest H. McCullough Jr., Tullahoma; his mother, Mrs. Jesse McCullough, Nashville; two sisters, Mrs. Gary Jones, Vivian Louisiana, and Mrs. Owen Frazier, Nashville, and a brother, the Rev.
