John Thomas Kuizinas of Decatur, Illinois United States was born on May 7, 1938 in Clinton, IN. He was married to Sheryle E. (Petersen) Shank on November 1, 1964 at Princeton Church, now Methodist Church - Had reception at Duck-N-Resort owned by Beryle (Petersen) Seelhorst, in Princeton, Green Lake County, Wisconsin and they later divorced in 1987 in Decatur, Macon County, Illinois. John Kuizinas died at age 70 years old on August 30, 2008 at 406 Sheridan Drive, in Decatur.
