Stephen Douglas Phay was the son of Stephen Rowley Phay (1824–1867) and Rebecca Crawford (1824-1915). He was born on March 12, 1861 in Dayton, Montgomery County, Ohio, USA and had siblings Elon James Phay (1847-1911), John Phay (1850-1935), Ella Phay Whitney (1855-1907), and Emmeline E Phay Zentz (1859-1915). He went by his middle name "Douglas" most likely due to the fact that his father's name was also Stephen. He married Ella Keziah Hollingsworth (1862-1937) on April 19, 1879 in Marshall, Iowa. They had two children together June Helen Phay Bloom (1884-1952) and Ralph Douglas Phay (1889-1947).
